Logging Injuries

What do you use to document injuries that happen in your care and injuries that happen at the child's home?

I'm having difficulty finding a book specifically for this.

I think CH has something that she uses.


I take pictures, send them to the cloud,and put as much detail in the label of the photo.

I also have to use a state form for reporting reportable injuries.

I have a 1 inch binder that I am required to keep for inspection. I saved this form Authorization to Dispense External Preparations (ga.gov) in PDF and fill it out, print two copies and have the parent sign both. They keep one, I keep one. I don't know why the link says what it does, but it goes to the right form. You can edit the form in any way you choose once it is in PDF.

For injuries upon arrival I use the "add note" section on my sign in tab (Daily Connect). It then stores it and emails it to both the parents and me at he end of the day. I can print from either source, or both, at will. Before that I kept a running journal in word and sent an email that they responded to. Print/Save.

My county licensing website has an injury report form they want us to turn in for injuries that require medical attention. What I do for small injuries or significant home injuries: is use that form and write "DOCUMENTATION ONLY" on the top and highlight it. Then put it in their file.

I sometimes have the parent sign it too. Especially if it's a behavioral related injury and termination might be in their future.

I just use ours for consistency. If I ever have a serious injury I have to log into the state account and self report within 24 hours. I'd probably just give my inspector a call after treatment was complete, too, just to hurry things along and keep it from becoming out of context with social media.

It's clearly a long week (if only in my mind) as I keep reading the title of this tread as if it's about injuries loggers receive.
I live in a heavily forested area with a lot of logging companies so...

I read it that way the first couple times, too. I'm picturing degloving's, crush injuries and compression injuries.

I never had to record an injurie that happened in my care (knock on wood)
I had a few bad injuries that happened while the kids were at home every time I received text and pics from the parents so I figured that was my record
