Form 3115

Hi. I am doing my taxes this year and want to file IRS Form 3115. Has anyone here tried this? I asked my local CPA if she would do this for me, she told me she has performed a 3115 only once in her 35 year career. She declined but told me she would "put her feelers out" and let me know if someone can help. She has a very good reputation. I've called Tom Copeland a couple of times about the form. He gives a great example of how to do it in his workbook. I filed an extension so I have until October to figure things out. Any tips would be greatly appreciated.
